I'm from upstate New York, where the surname "Campbell" is pronounced with the "p" and "Burrell" is pronounced "burr-ell" (in Philadelphia it rhymes with "girl")...

The Kaufmans were our neighbors growing up, and I assumed their last name was spelled "Coughman."

There's a good one-volume book by the late Stuart Berg Flexner titled "Speaking Freely: A Guided Tour of American English from Plymouth Rock to Silicon Valley" which has derivations and meanings of American words and phrases.
