Does the quoted sentence refer to both situations where the husband is in the house or he is not in the house (i.e. away from the house)?

It would depend on context who 'us' is.

If it is her and the husband, they must both be outside the house.

If it is her and someone else, then the husband is probably outside the house as well, because if he were in there they could raise him. He is not deliberately locking them out because he is a 'fool' not anything worse.

The likelihood is, the husband locked the door when he left and now he and his wife are stuck outside without a key.
(Though why she didn't have one.....)
