Pandemic Is Overwhelming U.S. Public Health Capacity In Many States. What Now?

https://www.npr.org/sections/health-shots/2020/07/28/894858475/pandemic-is-overwhelming-u-s-public-health-capacity-in-many-states-what-now

Testing, contact tracing and isolation are the tactics being used successfully to crush outbreaks in countries such as South Korea and Germany. But those places never had the level of case surges that many U.S. states are now experiencing.

"When you have something like this happening, there's no way that traditional testing and tracing is going to have any meaningful impact," Osterholm says. "I liken it to trying to plant your petunias in the middle of a Category 5 hurricane."

Others agree. At this point, there are just too many new infections occurring too quickly for underfunded, understaffed public health departments to effectively use testing and contract tracing, according to Dr. Jeffrey Engel, the senior adviser for the COVID-19 response for the Council of State and Territorial Epidemiologists.

"It's just this massive effort," he says. "It's just not feasible."

U.S. Tops 70,000 Coronavirus Cases In 1 Day — Heights Not Seen Since July
October 23, 2020 9:28 AM ET

https://www.npr.org/sections/coronavirus-live-updates/2020/10/23/927070899/u-s-tops-70-000-coronavirus-cases-in-1-day-heights-not-seen-since-july

The U.S. recorded 71,671 new coronavirus cases on Thursday, the most in one day since the outbreak hit alarming heights in July, according to data compiled by Johns Hopkins University. One day earlier, around 63,000 new cases had been reported.

The U.S. also recorded 856 deaths from COVID-19 on Thursday, raising the death toll to more than 223,000 people lost to the pandemic.

U.S. cases have been rising sharply since the middle of September, when the daily rate was hovering around 40,000 cases.

Public health experts are now watching to see if Thursday's results are an aberration or part of an ominous new pattern. When the U.S. hit a record of more than 77,000 cases in mid-July, it topped the 70,000-mark on several other days as well — and the numbers stayed worryingly high through the end of the month.

The US has now topped 9 million cases of covid19. This is an additional million cases in just 13 days.

Comparison to other countries:

We have indeed rounded the corner. It's not the corner you think. It's like when you drive through a new area and the thought crosses your mind "this would be a really bad place for my car to break down." For the math fans out there, the corner is a concave up inflection point.

Deaths recorded again in Iceland, after no deaths since the middle of April. So that is a bad trend. But tourist season is running down and if people are responsible it should be contained. Stricter lockdown starting from today.

People have been in favour of restrictions and border controls - despite press moaning about "freedom", questionnaires have shown this doesn't reflect the attitude of people, who understand the need to keep it out. As an island, you can do that if you are vigilant at the border and people are responsible about quarantining themselves before arrival. If they don't - well, I guess I will have to trust karma to fuck them up eventually.

Quote:
75 people were diagnosed with coronavirus domestically yesterday. 60–or 80%–of them were already in quarantine at the time of diagnosis, according to Vísir. 5 were diagnosed at the border.

A total of 64 are currently in hospital with the virus whilst 4 are in intensive care. There have now been 4,797 confirmed cases in Iceland since the start of the pandemic. Of those who have fallen ill with the virus, thirteen have now died–three of them in this third wave, and two of them in the past week.

The incidence of domestic infections (cases per 100,000) now stands at 213.3. New restrictions in response to the third wave were announced today, and go into effect at midnight tonight.

New total lockdown next week in the UK but I have to keep working, because colleges are staying open. d'oh!

Hawaii has been quarantining people since March. The state's economy is based on tourism. They have had a mandatory 14 day quarantine for all out of state visitors. The quarantine has devastated the economy. Many people have left to go back to the mainland to look for work. The quarantine has kept infections lower than many states, but hasn't eliminated them. They've still have had about 15,000 cases and just over 200 deaths. The quarantine was lifted on October 15th. All you need now is a negative covid19 test within 72 hours of your arrival. This, however, still leaves a fairly large window open for infectious people to enter the state. Other states have fared much worse. For example, California and Texas are both approaching a million cases each which is worse than many countries.

We're lucky in my family that everyone's work is computer-based so this 2nd lockdown won't be too bad. But it's going to wipe out some of my friends who own small businesses and were still struggling after the last lockdown. So many tears shed over the past few days and that depressing feeling of utter helplessness in the face of their tragedies.

For me personally it's a wee bit scary. In February I was booked in for surgery which the advent of Covid soon pushed to early '21. With hospitals dealing with this 2nd spike, it's utterly impossible to know how long it will be now. I'm just not the kind of person to 'rest a lot and take it easy' - especially when there's no time limit on how long I should be lolling around taking it easy. I find it utterly impossible to slow down...I even walk fast! I do realise the risk - but I DID try to 'take it easy' for a couple of weeks and found the toll it took on my mental health was too heavy.

In the face of what my friends above are going through it seems petty & selfish even to mention the matter to anyone.

I expect there are people raging and railing against this 2nd lockdown but there are others who were seriously alarmed by the rising infection numbers & body count and who were relieved that at least a decision had been made.

My own opinion is that we WERE treated as responsible adults, who would take precautions for the good of our country. But we didn't do that. We acted like arseholes. Which clearly explains why shelves are being stripped of toilet-paper once more.Whistle Dancing

U.S. Confirmed Coronavirus Infections Hit 10 Million
November 9, 2020 2:08 PM ET

https://www.npr.org/sections/coronavirus-live-updates/2020/11/09/933023659/u-s-confirmed-coronavirus-infections-hit-10-million

More than 10 million people have now been confirmed to be infected with the coronavirus in the United States as the spread of the virus accelerates at an alarming pace across the nation.

The U.S. now accounts for about one-fifth of all of the 50 million infections worldwide, more than any other nation. The data comes from Johns Hopkins University.

The U.S. also is also in the unenviable position of suffering more deaths than any other country. More than 237,000 people have died from COVID-19 in the U.S., and more than 1,000 are frequently dying on a daily basis.

"This didn't have to happen," says Jeffrey Shaman, an infectious disease expert at Columbia University.

The grim milestone comes as the number of new infections is setting records from coast-to-coast in the third and most widespread surge in reported infections since the pandemic began. And there are many more cases that have occurred but that have not been detected or confirmed, since many people are unaware they are infected.

It took only 10 days for the total tally to increase from 9 million. The U.S. is currently averaging more than 100,000 cases per day and could exceed 200,000 new cases per day by Thanksgiving.

"It's entered the exponential phase, and that means it's going to get worse and, frankly, a lot worse before it gets better," Dr. Thomas Frieden, a former director of the Centers for Disease Control and Prevention.

The Midwest is being hit the hardest, but no part of the nation is being spared.

The spiraling epidemic is pushing hospitals to the limit. Nearly 57,000 people are currently hospitalized with COVID-19.

The surge comes just before many people will travel and many families will gathering for the Thanksgiving and Christmas holidays, raising fears the spread could intensify even further. It also comes as the cold weather of winter will force people to spend more time indoors, which is the virus spreads the most. Flu season is also intensifying.

"This is a terrible time. The confluence of factors here is extremely problematic," says Frieden. "This is a very dangerous time."

But while "this is currently out of control, we still have an opportunity to control the pandemic," says Dr. Carlos del Rio, an infectious disease expert at Emory University.

But "we cannot wait until Jan. 21," says Del Rio, referring to the beginning of the Biden administration. The current administration should take more aggressive steps now, he says.

"We need a national mask mandate now," Del Rio says. "We all need to 'step up our game' and use a face mask all the time we are in a public place, practice social distancing, wash our hands and avoid crowded congregate and not well ventilated places. Wearing a face mask should not be a partisan issue, it should be a public health priority."

I just decided today to accept that Christmas is gone for us, as was our Thanksgiving. I have planned a really nice surprise for our son and wont' be there to see him open the envelope. 😢

Numbers of Covid are rising in Ontario exponentially, worse then the original stage in March+. Private parties are limited in number, but inconsistently, casinos, bars, and schools are open. The premier, Doug Ford, refused to provide money to lower class sizes so some classes have 35+ kids in small rooms. Makes no sense at all. Then Ford goes on TV and blames the people. Ontario has a self confessed Trump supporter idiot as the premier. Doug Ford, brother of deceased Rob Ford who was that terrible mayor of Toronto the world knew, is the premier of Ontario. He didn't finish high school, has a reputation for drugs in his youth, and is a supposed businessman.

He now takes federal billions while he rails against the suggestion of others that the Prime Minister of Canada should invoke the Emergency Measures Act to get control over Canada's citizens and the premiers of the provinces. It is the provinces with Conservative premiers that have the most Covid. Who could have forseen that? Whistle

Before the election Ford released no platform except for one where he was caught out. While the people are concentrating on Covid, he is now quietly going against what he promised before the election, and is allowing developers access to environmentally protected land. Before the election it came out he had been discussing it with developers but he back pedalled when there was outrage. Now he's going ahead while people are distracted. He actively dismantled half finished green projects, tried to sue the federal government against the federal carbon tax (where consumers were paid) and lost us 2 billion dollars in revenue by cancelling a Cap and Trade deal with California that had been started by the Liberal premier before the election.

It was dodgy how he got in to power - they accused the leader of the Conservative party at the time of sexual harassment - the woman has never been heard from since - and the party ousted that leader. Enough people were ready for a change and voted party anyhow.

Sound familiar Oscar?

Covid19 cases reach 11 million.

U.S. Hits 11 Million Coronavirus Cases, Adding 1 Million In A Week
November 15, 2020 9:24 PM ET

https://www.npr.org/sections/coronavirus-live-updates/2020/11/15/935229627/u-s-hits-11-million-coronavirus-cases-adding-1-million-in-a-week

More than 11 million confirmed coronavirus cases have been recorded in the United States, according to a COVID-19 tracker by Johns Hopkins University. The country reported 166,555 new cases on Sunday, with 1,266 new deaths.

The staggering milestone was reached only six days after the U.S. hit 10 million cases. Positive test rates and hospitalization rates are on the rise across the country, according to the Centers for Disease Control and Prevention.

Hospitalizations continue to climb. More than 69,000 people were hospitalized as of Sunday, more than ever before.

The pandemic also continues to disproportionately affect Black and brown communities in the U.S. According to data from the CDC as of Nov. 7, hospitalization rates for Hispanic or Latino people are 4.2 times higher than that of white people. American Indian or Alaska Native people have been hospitalized at 4.1 times the rate of white people, with Black people being hospitalized at 3.9 times the rate of white people.

As the country continues to grapple with the latest surge, many states are reinstating stay-at-home orders and restrictions. On Sunday, Washington Gov. Jay Inslee announced new restrictions for the state that include prohibiting indoor gatherings of people from more than one household. Also on Sunday, Michigan Gov. Gretchen Whitmer announced a three-week emergency order that will allow Michigan bars and restaurants to only be open for outdoor dining, carry-out and delivery.

Actions at the state level come as President Trump's refusal to concede has muddled the federal government's actions. The Trump administration has blocked the current coronavirus task force from communicating with President-elect Biden's team.

Dr. Anthony Fauci, the head of infectious diseases at the National Institutes of Health who is also on the current White House task force, appeared on CNN's State of the Union on Sunday. He said from a public health perspective, "it's almost like passing a baton in a race, you don't want to stop." He added, "it would be better if we could start working with them."

U.S. Passes 12 Million Confirmed Coronavirus Cases
November 21, 2020 5:06 PM ET

https://www.npr.org/sections/coronavirus-live-updates/2020/11/21/937615700/u-s-passes-12-million-confirmed-coronavirus-cases

Cases of COVID-19 are rising at an alarming rate in nearly every state as the nation approaches Thanksgiving.

The cumulative case count passed 12 million on Saturday, six days after the previous million mark was crossed, which was six days after the previous million.

On Friday alone, there were more than 195,000 new confirmed cases and 1,878 deaths reported, according to data from Johns Hopkins University.

More than 255,000 people have died in total, as of Saturday afternoon.

And more than 1,000 hospitals, across all 50 states, Washington, D.C., and Puerto Rico, say they are "critically" short on staff, according to the Department of Health and Human Services. And that number is expected to increase after the holidays.

To avoid an even starker increase in the spread of the virus, the Centers for Disease Control and Prevention is urging people to not travel for Thanksgiving at all. The CDC says Thanksgiving should be spent only with people actively living in your own household for at least 14 days before the holiday, advice many medical professionals are echoing.

"Travel may increase your chance of getting and spreading COVID-19," the CDC said on Thursday. "Postponing travel and staying home is the best way to protect yourself and others this year."
