Sounds like the person had a mouthful of food and said mouthful as "mirthful."

I knew Betsy as a youth. She simply snapped one morning. We were discussing her father's old war injury - the one that left him with an elbow that would only bend backwards - and she sat down and, simply snapped. It was very sad. I said, "Betsy, what's up with the mirth?" And she said, "Daddy's elbow problem sometimes cracks me up." ...And I knew the end had come. After all, this was her father's elbow we were discussing here.
