Hi again.

Would you recommend it be included. - "It be" is subjunctive.
Would you recommend it should be included. - I think you may hear this, but it is not correct. "Recommend" includes the idea of 'should'. When you say "It should be included" you are recommending the inclusion.
Would you recommend it will be included. This doesn't work. "Recommend" is possible, hypothetical - 'they' might not accept the recommendation.
"Will be included" is a definite, unquestionable future fact. They don't fit together.
Would you recommend it is included. - No. "Recommend" is followed by the agent (it) and bare infinitive (be).

Expressing Commands, Suggestions, Requests, and Statements of Necessity

When we express actions that we demand, suggest, or request that someone else take, or describe something that must be the case, we use the base form of the verb — that is, the infinitive form without the word to.
For example:

“He demanded that they leave the room at once.” (command)
“I recommend that she study harder next time.” (suggestion)
“I ask that the audience be completely silent during the demonstration.” (request)
“It’s necessary that we be vigilant to avoid another disaster.” (statement of necessity)

Drag0n hit the nail on the head as to reasons. Only the first sentence works:
Would you recommend it be included?

Other choices might be these:
Would you recommend its inclusion?
Would you recommend including it?

This sentence implies asking for a recommendation:
Should it be included?

This is closer to your original format:
Do you (think)(feel) it should be included?
