Before the test, all the participants were assured in terms of their privacy to remain confidential, and they all cooperated of their own volition and filled up testimonial forms.

Any improvements?

Prior to testing, all participants were assured of the privacy and confidentiality of their information. All participants also voluntarily co-operated and filled out testimonial forms.

What would be on this form? I would assume they completed a consent form. Or a medical history? Think

I don't understand 'testimonial form'.
To me 'testimonial' is:
saying something nice about someone,
a monument, being the clear result of something good that has been done
or
having a benefit match. Whistle

I was a bit confused by that "testimonial form" myself, but finally decided it was probably some kind of "rating" of the testing experience, whether it was a pleasant one, or difficult, etc. But that could be an incorrect assumption on my part.
