A) I could tell from his tone of voice that Ken was disappointed.

B) I could JUDGE from his tone of voice that Ken was disappointed.

Question 1: Do these two sentences mean the same thing? If not, why?

Question 2: What's the difference between 'tell' and 'judge'?

Question 3: tell from... = tell by ...? judge from ... = judge by ...?

Yes, they both say essentially the same thing. Using "tell" like this means you discern, you understand or come to a conclusion ( you judge) something.

In fact, I was going to say that one's choice is dictated by the person one is: someone who has confidence in their own powers of objectivity; who looks at things from every angle, who is completely logical; might say "I could judge from his tone of voice" because they really *were* qualified to make a professional judgement.

Most people use "I could tell from his tone of voice" because they're making an educated guess. That's not a rule or anything - that's just how it seems to work out.
