I work as a nurse in a close custody prison. Each time that prisoners celebrate this holiday, there is always so much confusion about whether or not they can take their medication with water before the sun goes down. Some of your brethren even say that the medication alone violates the fast if taken before sundown. Can you shed some light on this matter to clarify my own personal understanding so that I can be effective with my patients during this time?

I am not Muslim, but I have worked with Muslims who told me that fasting did not apply to those with medical conditions, breastfeeding or the young. And that medicine like injections is fine as long as it is not sustenance. It sounds like medicine with water comes under that list. But I am sure there are loads of resources out there like the website suggested or just the guys here, although you might need an authoritative source to reassure people they are not doing anything wrong.
