He is being rather cynical about doctors. Did he think they wanted to prolong the illness to earn more money?

Tough the writer meant sarcasm but sometimes it's the fact .For example in some types of cancer the only ting we can do is prolong the life of patient. Whereas in other diesses eg Diabetes Mellitus patients used to die couple of years . Physicians take an oath to benefit there patients not harm them, Beneficience non malifecence .

The human body doesn't come with a set of instructions, & all medications have side effects. Anything prescribed by a doctor is just a guess based on what's worked for other patients. Two (or more) diseases can have the same symptoms. An effective medication for most, can cause an allergic reaction in others. One patient could die for lack of penicillin, another patient could die from being given penicillin. Medicine is largely guesswork.
