How do I understand this?

1. Jack Swigert added emphasis (to whatever) choosing the Present Perfect Tense.
2. Emphasis was added in the film changing the original into the Past Simple.
3. The author of the Wikipedia article added emphasis italicizing the phrase.

I wonder if I missed any other possibility. :)

This is explaining the use of italics - obviously he didn't speak in italics.
They are added by the writer.
So, in reporting what was actually said, the writer of the article is drawing your attention to the 'had'. They are adding the emphasis.

When you expect to see one thing, it is easy to miss the difference in what is actually written. So the writer adds italics to point out the difference.
Then the explains that he (or she) has added the emphasis, and it wasn't an emphasis of the spoken word.
You have to report any alterations you have made to a quotation.

As he said it, it was normal, with normal stress on the past participle. Explaining what has just happened:
We've had a problem

Natural emphasis on the key word - problem.


If, when speaking, you strongly emphasise the 'had' you are emphasising it is in the past - it strongly implies you have fixed it.
We've had a problem but it is OK now.

But
Houston, we have a problem
Is far more immediate, dramatic. Imperilled.
And much better suited to becoming a cultural meme. Whistle
