From Richard Laymon's Midnight Tour:

(A man and his girlfriend are getting off a bus)

"Letting her go ahead of him, he realized that, right now, he didn't even like the way she looked from behind."

(Then follows a description about the actual hair and clothes of the woman.)

What does mean the "right now" in this sentence?

A, He right now realized that he doesn't like his girlfriend's backside (and never liked it).
B, He usually likes his girlfriend's backside but in this one instance he finds it repulsive.

right now - in this instant.

so your question is, which verb does it refer to?

a) right now he realised

or
b) right now he didn't like it

which phrase is it in?

If you take it away, the sentence splits like this:

"Letting her go ahead of him,he realized that he didn't even like the way she looked from behind."
"Letting her go ahead of him,
he realized that,
right now,
he didn't even like the way she looked from behind."

subject verb object
He realised that __________

so everything after 'that' is in the object phrase - what he realised.

"he realized that
right now, he didn't even like the way she looked from behind."

so it refers to him liking the way she looked.

If you want it to refer to the first verb, it would go in that first phrase. Before the 'that ____'.
eg
Letting her go ahead of him, he realised, at that moment, that he didn't even like the way she looked.

(I don't like the use of 'right now' with that verb, so I have changed it a bit for that example.)
