Grammatically correct, if you mean you always misinterpret someone.

Haven't heard anyone ever saying that. Maybe they all understand me well ;-)

It is just not something you would ever say. Whatever you are trying to express, it has to be done another way.

I suppose it could mean "I always mistake you for [someone else]," or possibly "I always misunderstand you."

But the original sentence as it stands just doesn't work.

There are several things which can be misunderstood about a person.
They are said in different ways.
"I mistake you" does not make sense to me.

"I misunderstand you." = "I mistook what you said."

"When I saw you, I thought it was your brother." = "I mistook you for your brother."

"Whenever I see you or your twin brother, I get the names wrong." - "I always mistake you & your brother.""

"My ideas about you were wrong." - "I had a mistaken idea about you."
