Gosh! Violent story, isn't it? I only read a little bit about the Jaguar killings.

It is saying that old Dr. Haskell had never even looked at the idea of a brain tumour because Junior Rennie was a healthy young man and was only about twenty years old.

"Otherwise" in this sentence means 'in every other way'.

He had a brain tumour, but the doctor didn't think of that because, in every other way, he was perfectly healthy.

The word "not", in this situation, has no effect on the meaning of the sentence. It merely adds emphasis to what follows it.

"Otherwise healthy" means that there seems to be nothing wrong with the young man's health, except that he has suddenly gone crazy for no apparent reason.

Think of 'otherwise' as meaning 'other ways'.


...an otherwise healthy man


Becomes

...a man [who is] healthy in all other ways

Ie, there is nothing else wrong with him, as explained.
