I am doing a drug history and need to document the following two ideas for the pharmacist to check:

The patient stopped Natalizumab 3 months ago. the patient thinks or believes not effective.

Can I use the reduced relative clause?

The patient stopped Natalizumab 3 months ago believing/thinking not effective.

I know it does not sound right but want to use present participle for my knowledge

This is grammatical and common:

"The patient stopped Natalizumab 3 months ago believing/thinking that it was not effective."

- The patient stopped Natalizumab 3 months ago, believing/thinking it not effective.

The participle (or rather the whole phrase that it introduces) modifies "the patient".

- "The patient, thinking the drug was not effective, stopped taking it three months ago."

- "Thinking the drug was not effective, the patient stopped taking it three months ago."

I have nothing to add about your present participle phrase (reduced clause).

NKM has answered well.

However, your "full version" also needs correction.

The patient stopped Natalizumab 3 months ago. the patient thinks or believes not effective.

The patient stopped Natalizumab 3 months ago. The patient thinks or believes that it was not effective.
